Background IMN tibial fracture repair via suprapatellar approach is an acceptable modality and highly popular surgical technique in case of proximal diaphyseal tibia fractures. Hypothesis Suprapatellar IMN approach is considered safe with reduced anterior knee pain for the patient. Objectives To describe a rare complication of an Intra-Medullary Nailing via suprapatellar approach with the following evaluation and treatment. Results The patient underwent intramedullary nailing via suprapatellar approach according the AO surgical technique. Postoperatively, the patient complained on anterior knee pain at the site of the surgery. On CT and arthroscopy, debris were found in the joint space. A secondary procedure was needed to prevent infection and further damage to patient’s knee. Conclusions In this case report we emphasize the potential damage to the articular surface of the knee and present to you a complication which should be kept in mind for the operating staff, with a notion for future prevention.
